Privacy & HIPAA Notice
How we handle your information, on this website and in our office.
Website privacy
What this website collects
Browsing the informational pages of this site does not require you to identify yourself. Our web server records standard technical information — IP address, browser type, pages requested, and timestamps — for security and for keeping the site running.
This site loads a web font from Google Fonts and, on the contact page, an embedded map from Google. Those third parties may receive your IP address when their content loads. We do not run advertising trackers on this site.
Patient forms submitted through this site
The patient forms collect protected health information (PHI). When you submit them:
- The information travels to our server over an encrypted HTTPS connection.
- It is assembled into a PDF packet and stored encrypted at rest on a server we control.
- Clinic staff retrieve the packet from a password-protected area. The packet itself is never sent by ordinary email.
- Submitted packets are retained as part of your medical record in accordance with Missouri record-retention requirements.
While you are filling the form out, a draft is kept in your own browser’s local storage so you can finish later. That draft never leaves your device until you press submit, and you can clear it at any time from the form itself.
Email is not secure
Please do not send medical information, prescriptions, or other PHI to us by ordinary email. Email is not an encrypted channel and we cannot guarantee its privacy. Use the patient forms or call us at 636-333-2235.
Children
This website is not directed at children under 13, and we do not knowingly collect information from them through it. Minors are treated in our practice only with the consent of a parent or guardian.
Notice of Privacy Practices
Effective date: September 1, 2026
This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
Our commitment
Arnold Medical Weight Loss, LLC is required by law to maintain the privacy of your protected health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we may use and disclose your health information
- Treatment. To provide, coordinate, and manage your care — for example, dispensing and shipping your medication, or sharing records with your primary care provider.
- Payment. To obtain payment for the services we provide, including processing your card payment.
- Health care operations. For quality assessment, staff review, business planning, and administration of the practice.
- As required by law. Including public health reporting, health oversight activities, judicial and administrative proceedings, and law enforcement requests where the law requires disclosure.
- To avert a serious threat to your health or safety, or that of another person.
Most other uses and disclosures — including any use of your information for marketing, and any sale of your information — require your written authorization, which you may revoke at any time in writing.
Your rights
- Inspect and copy your medical and billing records, in paper or electronic form.
- Request an amendment if you believe information in your record is incorrect or incomplete.
- Request an accounting of certain disclosures we have made of your health information.
- Request restrictions on how we use or disclose your information. We are not required to agree to every request, but we must honor a request to restrict disclosure to a health plan for services you paid for in full yourself.
- Request confidential communications — for example, that we call you only at a particular number.
- Receive a paper copy of this notice on request, even if you agreed to receive it electronically.
- Be notified if a breach occurs that compromises the privacy or security of your information.
Telemedicine
Telemedicine visits are conducted over a video platform that supports HIPAA compliance and with which we maintain a business associate agreement. The same privacy protections that apply to an office visit apply to a virtual one. Visits are not recorded unless you are told otherwise and consent in advance.
